This document is a BIR employment-application form (Statement of Relatives Form). The applicant states the position applied for and preferred places of assignment, declares whether they have relatives in the BIR (naming them and checking/encircling the relationship), and swears under penalty of perjury that if a relative relationship exists with the Head of Agency, Appointing Authority, Recommending Official, or Immediate Supervisor under Civil Service rules, they are willing to have their appointment revoked. The form also includes a waiver agreeing to assignment to any BIR office and compliance with BIR rules and orders.
