03 Reference Release for Applicants

This is a Bureau of Internal Revenue job-application form, not a tax issuance. An applicant for employment with the BIR authorizes any company, person, or educational institution listed as a reference to disclose information about the applicant's qualifications. The applicant also releases the BIR and its employees, directors, officers, and successors, as well as the listed references, from liabilities, claims, and causes of action for providing or receiving that information, and agrees not to file a lawsuit or assert a claim. The form provides fields for employment, personal, and educational references and is completed with the applicant's full name, signature, and date. It contains no tax provisions.
