Annex A-2 is a blank Bureau of Internal Revenue template titled 'List of Approved Formal Charge/Closing Report' maintained by the Internal Investigation Division/Regional Investigation Division (IID/RID). It is a dated list (As of a specified date) of formal charges approved against subject personnel and their closing reports, with columns for the complaint reference number (CRN), the date the formal charge was approved, the name and position/place of assignment of the subject personnel, the nature of the offense charged, the date of approval/implementation of the closing report, and the date of transmittal of the formal charge to the Personnel Adjudication Division (PAD)/Legal Division (LD). It is prepared by the Chief, IID/RID and submitted by the Regional Director.
