Annex A-3 is a blank Bureau of Internal Revenue (BIR) reporting template for a Revenue Region's Regional Investigation Division. It is a list of administrative cases — characterized as grave or less grave offenses — forwarded to the IID. For each case the form records: complaint reference number (CRN), name of subject personnel, position/place of assignment, nature of offense, date of transmittal to IID, and remarks. It is prepared by the Chief, RID and submitted by the Regional Director. The text contains no tax rules, rates, or taxpayer obligations; the Revenue Region number and 'as of' date are left blank.
