Annexes C1 and C2 are evaluation report templates for the 'Taxpayer Awareness Program' operational key performance indicator. Annex C1 covers the 1st semester and Annex C2 covers the calendar year. Each report scores a Revenue District Office (RDO), with the score based on a KPI Weight of 3.00 per RMO No. 25-2022 and computed from the RDO's Average Points per Month. Reports are prepared by the Chief, Internal Communications Division and certified by the ACIR, Client Support Service.
