This is a one-page table listing 20 bank branches for Revenue District Office No. 99 - Bukidnon under Revenue Region No. 16 - Northern Mindanao. Each entry shows an item number, bank code, bank name (abbreviated, e.g., BOC, BPI, LBP, PNB), branch name, and address. Based on the document title it appears to be a list of Authorized Agent Banks (AABs), but the extracted text itself does not state the banks' function, any effective date, or an issuance number.
