RMC No. 103-2023 (issued October 4, 2023) notifies the loss of two (2) sets of unused/unissued BIR accountable forms: BIR Form No. 0535 – Taxpayer Information Slip (Serial No. TIS201900271646, 1 set) and BIR Form No. 0423 – Apprehension Slip (Serial No. APS200300034671, 1 set). The forms were reported lost by Revenue Officer Cherry S. Wong of Revenue District Office No. 4 and have been cancelled; all official transactions involving their use are considered invalid.
