The loss was reported by four revenue officers of Revenue District Office No. 39 (Susan C. Alariao, Reymund S. Bumanglag, Shaira Mae G. Cortez, Eliza Victoria C. Nacional). The listed forms have been cancelled, and all official transactions involving their use are considered invalid.
