This circular announces that thirty-two (32) sets of unused/unissued BIR Form No. 0423 - Apprehension Slip bearing specified serial numbers were reported lost by a Revenue District Officer and have consequently been cancelled. All official transactions involving the use of the lost forms are considered INVALID. Internal revenue officials, employees and others concerned are asked to promptly notify the Office if the forms are found and to prevent improper or fraudulent use of the same.
