This circular gives notice that five (5) pads of unused/unissued BIR Form No. AF51 (Accountable Form No. 51), bearing specified serial numbers, were reported lost by the OIC-Chief of the Administrative Section of Revenue District Office No. 75, Revenue Region No. 11 (Western Visayas). The lost forms have been cancelled, and all official transactions involving their use are considered invalid. Internal revenue officials, employees and others concerned are asked to promptly notify the Bureau if the forms are found and to take measures to prevent their improper or fraudulent disposition or use.
