This circular gives notice that various copies of used/issued accountable forms were damaged by Typhoon Kristine: two (2) triplicate copies of BIR Form No. 1918 (Authority to Release Imported Goods) with serials ATR200900108101 and ATR200900108102, one (1) triplicate copy with serial ATR200900108104, and one hundred (100) duplicate copies of BIR Form No. 0535 (Taxpayer Information Sheet) with serials TIS201800129751 to TIS201800129850. All official transactions involving the original/taxpayer's copies of these forms should be verified, and BIR officials/employees concerned must take measures to prevent improper or fraudulent disposition or use of the damaged forms and promptly notify the BIR of any such event.
