RMC No. 65-2026 (issued June 22, 2026) notifies all internal revenue officials, employees and others concerned that two (2) sets of unused/unissued BIR Form No. 0423 - Apprehension Slip bearing Serial Numbers APS200300042484 to APS200300042485, reported as lost by Revenue Officer Rodolf Schervier S. Valerio of Revenue District Office No. 6, have been cancelled, and all official transactions involving the use thereof are considered as invalid.
