RMC No. 67-2026 (issued June 22, 2026) announces that several BIR Form No. 0535 – Taxpayer Information Sheet (TIS) forms were reported lost by Revenue Officer Glydelle D. Baguilat of Revenue District Office No. 8. The lost items are five (5) unused/unissued sets (TIS202500435939–TIS202500435943), seven (7) used/issued duplicate copies (TIS202500435932–TIS202500435938), and one (1) used/issued duplicate copy (TIS202500213110). The unused/unissued sets have been cancelled and any official transactions using them are invalid, while official transactions involving the lost duly issued copies should be verified.
