This circular notifies internal revenue officials, employees and others concerned that the triplicate copy of a used/filled-up but unissued BIR Form No. 0402 – Authority to Cancel Assessment (ACA) bearing Serial Number ACA202000015466 was reported missing by Revenue Officer Jhillian Lei C. Cabrera of Revenue Region No. 8A – Makati City. The entire ACA set bearing that serial number has been cancelled, and all official transactions involving its use are considered invalid.
